Well there was one last month (Keith vs Fort William) with PKs at 78, 87, and 89 minutes (all to the same side) that turned a 1-2 deficit to a 4-2 win.

Hard to tell. The camera angles and clip quality aren't great. It does seem like the defender pulls the attacker going for the ball down on top of himself. Perhaps the telltale sign is that the attacker jumps up but seems to stop rising a short distance off the ground and comes crashing down while the defender behind him never goes for the ball. On the other hand, the attacker pushes back and arches his body backward as he jumps up, and he could be falling onto the defender (and Cannon on top of them). What a mess. The ref doesn't hesitate at all if that means anything.
