As far as I can tell (my info may be out of date), although there have been several occurrences of a batsman reaching 50 entirely in boundaries, only one batsman in the history of first-class cricket has ever completed innings of 50+ without running:

At the risk of sounding like a boring old fart, one of the interesting (to me) factors about such a list is that it doesn't necessarily contain the world's greatest big hitters. For example, the name John Crawley features prominently in the 'fewest scoring shots to reach 50' list (51 comprising 12 boundaries and a three for Lancs v Derbys at Manchester in 1996) but Afridi doesn't.

I wonder if it comes down to the real sloggers being more likely to get mishits past the infield so still getting runs off them.
