Say, a bowler bowls a leg stump (or a little outside) line to a right-handed batsman. It is possible that playing a shot towards mid-wicket or the leg side may be risky (quite a few fielders, hitting against the spin etc.). So the batsman decides to play a shot towards mid-off or extra-cover. Shuffling a little towards the leg-side, he brings down an angled bat very near his legs/pads and strokes the ball towards the off-side. Usually employed against leg-spinners.
One such player I recall who was very good at playing "inside-out" shots was Reg Simpson of Nottinghamshire and England.

VVSL used it with tremendous effect against Shane Warne in his 281 at Eden Gardens.
